Terms of Service & Membership Agreement

1. Agreement & Operating Entity

These Terms of Service apply to the Lavara Rx website and the programs offered through it. The operator is Online Edge Agency LLC d/b/a Lavara Rx, 2031 Harrison St, Hollywood, FL 33020.

2. Telehealth Facilitation Role & Medical Disclaimers

Lavara Rx is an administrative, non-clinical technology platform facilitating access to telehealth care. Lavara Rx is not a medical practice, healthcare provider, or pharmacy, and does not practice medicine. Clinical healthcare services, medical assessments, diagnosis, and prescribing decisions are rendered independently by licensed physicians and medical groups through our affiliated network. No clinician-patient relationship is created with Lavara Rx itself.

3. Medical Intake, Prescriptions & Compounded Medications

Submitting a health-history form or paying a fee does not guarantee a prescription. A clinician licensed in the patient’s state must complete a live audio or video visit first. FDA-approved products are used when they meet the patient’s need. A compounded medicine is dispensed by Rush Pharmacy only when the prescriber documents a patient-specific need that an approved product cannot meet. Compounded medicines are not FDA-approved. 4. Subscription Billing, Automatic Renewal & Cancellation

• Recurring Monthly Membership: Clinical programs are offered as recurring monthly subscriptions. By starting a program, you authorize recurring monthly charges to your payment method on file until paused or cancelled. • Cancellation Without Penalty: You may pause or cancel your subscription at any time without fees by emailing support@lavararx.com or visiting your patient portal. To avoid renewal charges, cancellation requests must be submitted at least 48 hours prior to your monthly billing date.

5. Refund & Return Policy

Under federal and state law (including Board of Pharmacy regulations), prescription drugs cannot be returned, restocked, or resold once dispensed and shipped. Therefore, all medication sales are final upon pharmacy shipment. If a clinician determines you are medically ineligible for therapy during intake evaluation, you will receive an immediate 100% refund of any initial consultation fee.
